Schedule: Business hours Monday through Friday, with occasional on-call responsibilities

Pay: Starting at $75k per year for LPNs, and starting at $98k per year for RNs

 

About the Position

The Assisted Living Manager is a hands-on, working nurse providing care and oversight to residents with a wide variety of clinical needs. This position supports the safety, health and wellbeing of residents living in Assisted Living. Assesses residents and participates in development of Individual Service Plans, oversees the delivery of appropriate services to residents that allow a maximum level of independence in the congregate setting, and maintains close working relationship and open lines of communication with all departments to fulfill the psychosocial needs of residents, as well as clinical ones. 

Key Duties

  • Coordinates an inter-disciplinary approach to the physical, emotional, psychological, familial, spiritual, social and nutritional needs of residents
  • Implements Individual Service Plans and communication of such to care team members. Attends service plan meetings and provides nursing updates, including medication review. 
  • Manages Activities of Daily Living (ADL) care provided by a team of caregivers including performing daily rounds for supervision of care, coordinating specific resident/family care requests, customer service feedback and resolution, and managing transportation to/from on-site appointments.
  • Manages caregivers in Assisted Living, with input from Wellness Nurses and other Interdisciplinary Team (IDT) Members for hiring, training, scheduling, performance assessment and evaluation, and coaching and discipline as needed.
  • Provides supervision to and works collaboratively with the Wellness Nurses to provide clinical services in an Assisted Living setting, including daily hands-on participation in workflows to support completion of clinical activities. 
  • Serves as clinical liaison to healthcare professionals and families, as appropriate.
  • Maintains open communication and positive relationships with residents, families, volunteers, staff and other stakeholders.
  • Reviews the 24-hour report, new orders, new admissions, and other relevant sources to provide a comprehensive clinical review to Director of Nursing and IDT during daily clinical meeting. 
  • Participates in completion of documents, including incident reports, scheduled and change-of-condition resident assessments and evaluations, and admission documentation.
  • Audits admissions, re-admissions, transfers, discharges, incident reports, new orders, scheduled assessments and electronic medical record (EMR) dashboards for regulatory compliance, completion and accuracy. Takes steps to remedy inaccurate or incomplete information. 
  • Manages assisted living processes to ensure compliance with regulatory standards, including completion of scheduled weights and coordination with dietitian, monthly medication review and semiannual resident reconciliation, wound management in partnership with Home Health and Wound Physician, scheduled resident assessments and evaluations, and notification of provider and responsible party as required. 
  • Seeks to ensure compliance with regulations including maintaining code cart, refrigerator temps, narcotic count documentation, equipment tracking, etc.
  • Provides oversight of Registered Medication Aides with a focus on accurate administration of medications and ongoing education of pharmacology.
  • Maintains a focus on safety and risk management at all times. 
  • Must be able to work flexible schedule, including periodic Manager-on-Duty shifts, nights, weekends, and on-call as needed. 
 

Job Requirements

  • Must be a graduate of an approved RN or LPN program.
  • Must have current licensure in the Commonwealth of Virginia or compact state of residence.
  • Must possess prior supervisory experience. 
  • Minimum of 5 years clinical nursing experience with a geriatric population.
  • Must have current CPR certification.
  • Must possess ability to deal tactfully with residents, staff and family members.
  • Excellent ability to read, write, speak and comprehend the English language.
  • Proficient in using computers (Microsoft Office suite, Microsoft Teams, etc.) and Web-based platforms for secure management of resident and team member information.
