About the Position
The Director of Hospice Admissions is responsible for managing and leading the admissions department, ensuring a seamless experience for patients and families entering hospice care. This role involves overseeing the admissions process, establishing and nurturing referral partnerships, ensuring regulatory compliance, and fostering a compassionate approach to patient and family support. The Director will lead an admissions team and work closely with the interdisciplinary hospice team to ensure efficient and empathetic care coordination.
Job Duties
Admissions Process Management:
• Direct and oversee the daily operations of the admissions department.
• Coordinate with patients, families, and healthcare providers to ensure smooth transitions and accurate assessments.
• Develop and implement policies to optimize the admissions workflow.
Referral Development and Relationship Building:
• Build and maintain strong relationships with hospitals, nursing facilities, physicians, and other referral sources to promote the hospice program.
• Identify new referral opportunities and represent the hospice at community events and professional meetings.
Team Leadership and Development
• Recruit, train, and supervise admissions staff, ensuring they are equipped with the necessary knowledge and skills to provide compassionate service.
• Foster a positive, supportive team environment and provide ongoing feedback and development opportunities.
Regulatory Compliance and Quality Assurance
• Ensure that all admissions practices adhere to Medicare, Medicaid, and other relevant regulatory guidelines.
•Conduct regular audits to maintain compliance and improve documentation and quality of admissions processes.
Performance Monitoring and Reporting
• Track key admissions metrics, identify trends, and evaluate department performance.
• Prepare and present reports to senior leadership, focusing on admissions volume, referral patterns, and team efficiency.
Patient and Family Support
• Serve as a resource for patients and families, addressing questions and concerns with empathy and professionalism.
• Ensure all communications align with the hospice’s values of dignity, respect, and compassionate care.
Interdisciplinary Team Collaboration
• Coordinate closely with the hospice care team, including nurses, social workers, and other clinical staff, to align patient care plans with admissions requirements.
• Facilitate effective communication across departments to optimize patient and family support.
Qualifications:
- Education: Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field; Master’s degree preferred.
- Experience: Minimum of 5 years of experience in hospice admissions or healthcare administration, with leadership experience preferred.
- Knowledge: In-depth knowledge of hospice regulations, including Medicare, Medicaid, and other payer requirements.
- Skills: Strong communication, interpersonal, and organizational skills; compassionate approach to patient and family needs• Technical: Proficiency in electronic medical records (EMR) and admissions management software.
