Location: Alexandria, VA

Shift: 7:00 am to 7:00 pm

Pay: Between $34 and $38 per hour depending on experience

 

About the Position

This position is primarily responsible for monitoring the clinical and holistic health of the residents and delivering care utilizing the nursing process of assessment, planning, implementation and evaluation, while maintaining the standards of professional nursing. The LPN provides person-centered care in collaboration with care partners, medication aides and interdisciplinary team.

 

Key Duties

  • Function as a clinical resource person who coordinates communication with the Physician in order to direct resident care on a designated shift.
  • Assign tasks to Care Partners and/or Med Techs in accordance with care plans and individual service plans.
  • Learn resident preferences and support fulfillment of them. 
  • Assists the Director of Nursing in disciplinary problems and performance appraisal goal setting.
  • Coordinate staffing for assigned schedule and find replacement for call offs.
  • Provides leadership by being a role model and maintains effective communication with and feedback from employees in department.
  • Encourages feedback from, maintains relations with, and monitors commitment of providing highest quality service to customers including residents, internal customers, and partners/suppliers.
  • Assesses each resident for unresolved/untreated pain. Plans and implements clinical interventions when appropriate, especially related to pain management, skin integrity, bowel and bladder status, including continence and bowel movement monitoring, mentation, ambulation status, nutrition and hydration, resident safety and the reduction of injury and the effects of medication.
  • Consistently and accurately completes documentation and provides follow-up within reasonable time frames and standards of care.

 

Job Requirements

  1. Must be a graduate of an approved RN or LPN program
  2. Must have current licensure in the Commonwealth of Virginia or compact state of residence.
  3. Must have current CPR certification.
  4. Must possess ability to deal tactfully with residents, staff and family members.
  5. Excellent ability to read, write, speak and comprehend the English language.
  6. Must be able to utilize EHR.
