About the Position

Evaluates residents, under physician referral, with swallowing and communication disorders.  Develops effective treatment plans and obtains approval for services from referring physician.  Manages schedule per scheduling policy, communicating any issues to Director of Rehabilitation and rehab MDS coordinator as per policy. Completes all required assessments for Medicare skilled/Medicare A and Medicare B clients.

Communicates and teaches families and other health team members to improve outcomes and follow-through with compensatory strategies and diet changes.  Conducts in-services and screens residents as assigned to determine those at risk and in need of services.

Key Duties

  • Provides Speech Therapy evaluations and treatments to overcome specific losses of communication and swallowing abilities due to disease and trauma.
  • Effectively communicates with the physician, nursing, and other team members to relay resident’s progress with treatment, through accurate treatment notes, progress reports, fourteen (14) day and monthly re-evaluations, and discharge summaries per the Rehab Department’s policies and procedures and Medicare guidelines.  Copies progress note to the IDT record.
  • Coordinates resident’s treatment with nursing staff, dietary department, occupational, and physical therapists.
  • Reports changes in the resident’s medical status to the charge nurse for communication with the Physician.  Documents changes in IDT notes per department policy.
  • Participates in care plan meetings and discharge planning for patients on caseload, as assigned during the Friday care plan meeting.  Monitors the care plan list and gives a report to the therapist going to the care plan, if not assigned to attend, for all patients on caseload.
  • Maintains accurate billing records of treatment minutes for PPS billing, units and treatment codes for Medicare B billing, and per facility protocol for Managed Care clients.
  • Maintain treatment schedules of patient’s under their POC, and coordinates treatment times with other treating disciplines on a daily basis.
  • Communicates with rehab department MDS coordinator if unable to see any patients on the schedule, or if treatment time is less than called for on the POC..
  • Attends department’s Medicare A meeting weekly if possible.  If the meeting time conflicts with scheduled clients, gives an update on all patients before the meeting.
  • Communicates initiation of Restorative Nursing Programs to Staff Development Coordinator per established facility protocol. Trains floor nursing staff in resident specific programs.
  • Participates in per review process for yearly ST competencies.
  • Participates in QA studies as assigned by the Director of Rehabilitation.

Job Requirements

  • Master of Science or Master of Arts degree from accredited college or university
  • Certificate of Clinical Competence from the American Speech-Language Hearing Association
  • Current Virginia State Licensure
  • Maintains yearly CPR certification (adult only)
  • Possess the ability to deal tactfully and compassionately with residents, family members, caregivers, and co-workers.
  • Have the ability to motivate residents to participate as fully as possible in their rehabilitation programs.
