About the position
The Telephonic PRN Care Coordinator supports the Goodwin Living At Home program by assisting members through telephone and virtual communication. This role focuses on non-clinical care coordination, documentation, follow-up, and support tasks that can be conducted remotely. The Care Coordinator will ensure timely responses to member needs, maintain documentation in My Junna, and provide continuity of service during times of increased demand or staff absence.
Job Duties
- Maintain regular telephonic communication with members, family members, and providers to support care coordination and service navigation.
Document all communications, follow-up actions, and service updates in the electronic record system (My Junna) in a timely and accurate manner.
Conduct wellness check-in calls, post-discharge follow-ups, and quarterly outreach calls as assigned.
Assist with scheduling services, clarifying benefits, and coordinating care logistics through phone and email communication.
Coordinate with on-site or field-based team members needs or follow-ups.
Monitor alerts, voicemails, and emails when working to ensure responsive care coordination and follow-up.
Participate in interdisciplinary team huddles or meetings virtually, contributing insight and support when requested.
Uphold documentation and privacy standards in accordance with HIPAA and Goodwin Living At Home policy.
Support after-hours and weekend on-call needs as scheduled or requested.
Maintain working knowledge of available services, vendors, and policies to appropriately guide members and families.
Other tasks as assigned by Manager, Member Experience or Executive Director
Qualifications
- Bachelor’s degree in social work, nursing, gerontology, human services, or a related healthcare field preferred.
Prior experience in working with older adults in a community-based or home care setting is strongly preferred.
Exceptional verbal communication skills and telephone presence.
Strong documentation and time management skills.
Experience with EMR systems (preferably My Junna), Microsoft Office, and cloud-based communication tools (Zoom, Teams, etc.).
Ability to work independently, stay organized, and adapt to shifting priorities.
Demonstrates cultural competence and sensitivity to the needs of older adults.
Must be able to maintain a confidential and distraction-free home workspace.
Must be able to work flexible hours including rotating the after-hours on-call schedule and taking care of members’ needs night and weekends occasionally.
