About the Position
This position provides leadership, operational and clinical support to the hospice team.
Job Duties
o Team management: Responsible for overseeing and managing the clinical team, which may include nurses, social workers, counselors, and other healthcare professionals. This involves hiring, training, scheduling, and evaluating staff members, as well as providing ongoing support and guidance.
o Patient and Family Communication: Serve as the primary point of contact for patients and their families, ensuring clear, empathetic, and timely communication regarding care plans, changes in condition, and services provided. Address concerns, questions, and feedback from patients and families promptly, ensuring that all issues are resolved in a manner that enhances patient satisfaction and trust.
o Care coordination: Ensures the coordination and continuity of care for patients receiving hospice services. This includes collaborating with interdisciplinary teams to develop and implement care plans, conducting regular case conferences, and addressing any issues or concerns that arise during patient care.
o Compliance and Quality Assurance: Responsible for ensuring that the assigned team adheres to all applicable regulations, standards, and policies. This involves monitoring and evaluating the quality of care provided, conducting audits and assessments, and implementing corrective actions as needed to maintain compliance and improve service delivery.
o Education and Training: Identifies areas of education and training to the clinical staff, keeping them updated on best practices, new policies, and emerging trends in hospice care. This includes organizing in-service training sessions, workshops, or professional development opportunities to enhance the knowledge and skills of the team in collaboration with the educator.
o Performance Management: Oversees the performance of the clinical team, monitoring productivity, efficiency, and outcomes. This involves setting performance expectations, conducting regular performance evaluations, and providing feedback and support to enhance individual and team performance.
o Documentation and Record Keeping: Ensures accurate and timely documentation of patient care, including assessments, care plans, progress notes, and other required records.
o Continuous Improvement and Program Development: Participates in quality improvement initiatives and contributes to the development and implementation of new programs or services that enhance the overall hospice care delivery and meet the evolving needs of patients and their families.
Job Requirements
- A RN who is currently licensed by the Commonwealth of Virginia Department of Health Professional. Minimum 5 years’ experience as a professional nurse or clinical social worker, preferably in a hospice and palliative care setting.
- Certified Hospice and Palliative Care, preferred.
- Demonstrated leadership and organizational ability with a motivation to assist in the implementation of program development.
- Demonstrated ability to note and respond to the needs of terminally ill residents and families in varied settings.
- Ability to cope with family emotional stress and tolerance of individual lifestyles.
- Demonstrated computer skills for documents, spreadsheets and database management and ability to learn new electronic medical record software and become a trainer in health record documentations,
