About the Position

The Hospice RN Quality Coordinator is responsible for supporting the quality assurance and performance improvement (QAPI) activities within the hospice program. This position ensures that all care provided meets regulatory standards and best practices while supporting the ongoing monitoring, analysis, and improvement of patient care processes.

Job Duties

 Quality Assurance & Improvement:

  • Conduct regular audits of patient care records to ensure compliance with hospice care standards, policies, and procedures.
  • Identify areas of improvement and participate in the development and implementation of quality improvement initiatives.
  • Monitor key quality indicators and assist in the preparation of reports for QAPI meetings.
  • Collaborate with interdisciplinary teams to address any identified deficiencies in patient care and implement corrective actions.

Regulatory Compliance:

  • Ensure compliance with all federal, state, and local regulations governing hospice care.
  • Maintain up-to-date knowledge of regulatory changes and industry best practices related to hospice care.
  • Assist with preparing for and participating in external audits and surveys, including those by accrediting bodies.

Education & Training:

  • Provide education and training to hospice staff on quality improvement processes, regulatory requirements, and best practices.
  • Assist in developing educational materials and training programs to enhance the quality of patient care.

Patient Care Coordination:

  • Work with the hospice care team to ensure that patient care plans are aligned with quality standards and are consistently followed.
  • Participate in case reviews and ensure that patient needs are being met in a timely and effective manner.

Data Collection & Analysis:

  • Assist in collecting and analyzing data related to patient care quality and outcomes.
  • Use data to identify trends, areas for improvement, and the effectiveness of quality initiatives.

Job Requirements

  • Bachelor’s degree in Nursing required.
  • Minimum of 5 years clinical experience with a minimum of 3 years clinical experience in Hospice or Home Health required.
  • Demonstrate effective leadership skills in QAPI management, strategic planning, and/or program and staff development.
  • Demonstrate computer skills for documents, spreadsheets and database management and ability to learn new electronic medical record software and become a trainer.
