About the Position

The Hospice Team Assistant provides administrative and clerical support to the hospice care team, ensuring the efficient operation of hospice services. This role involves coordinating patient care activities, managing patient records, communicating with patients and families, and assisting with various administrative tasks to support the overall functioning of the hospice team.

Job Duties

  • Provide administrative support to the hospice care team, including scheduling appointments, maintaining calendars, and coordinating meetings.
  • Prepare and maintain patient charts and records, ensuring all documentation is accurate, up-to-date, and filed correctly.
  • Assist in preparing and distributing reports, correspondence, and other communications as needed.
  • Schedule and track transportation and other logistics for Goodwin Hospice patients to medical appointments and symptoms management situations.
  • Enter and update patient information in electronic medical records (EMR) systems and other databases as required.
  • Track and monitor patient care activities, ensuring compliance with hospice policies and procedures
  • Serve as a liaison between patients, families, and the hospice care team, ensuring clear and timely communication.
  • Coordinate the delivery of supplies and equipment to patients’ homes as needed.
  • Assist with the admission, transfer, and discharge processes, including the preparation of necessary paperwork and communication with all relevant parties.
  • Assist with maintaining office supplies, equipment, and inventory, ensuring that the hospice office is well-stocked and organized.
  • Answer phone calls and respond to inquiries from patients, families, and other healthcare providers, directing calls to the appropriate team members as necessary.
  • Support the hospice team in maintaining a compassionate and supportive environment for patients and families.
  • Ensure compliance with all federal, state, and local regulations related to hospice care.
  • Assist in preparing for audits and regulatory reviews by maintaining accurate records and documentation.

Job Requirements

  • High school diploma or equivalent required; some college or additional training preferred.
  • Previous experience in a healthcare setting, preferably in hospice or palliative care.
  • Strong communication skills, both verbal and written.
  • Excellent organizational and multitasking abilities.
  • Experience with electronic medical records (EMR) systems and basic office software (e.g., Microsoft Office Suite).
  • Ability to maintain composure in stressful situations and handle sensitive issues with care.
  • Strong interpersonal skills and the ability to work effectively with diverse groups of people
